a copy of the license from the Agency for External Labor Migration under the Ministry of Employment and Labor Relations of the Republic of Uzbekistan (for inviting persons to carry out labor activities) - a copy, 1 piece;
a sample letterhead of the organization indicating the legal address, phone numbers and other contact details;
a sample signature of the head of the organization and an imprint of a round seal on a letterhead (+ a copy of the head's passport + a copy of the order for the head);
power of attorney for an employee of the organization (passport + copy of the order on his appointment) involved in issuing a military visa on letterhead - 2 pcs. in original.
